I have created a trigger on attachment object i am performing some operations in it ( on attachment after insert ). The trigger is getting execute successfully in salesforce classic mode but when i change mode from classic to lightning the trigger is not going to execute.
The triggers on other objects are running successfully in lightning. I am facing issue with this attachment object only.
Is there any solution to overcome this issue?
While uploading a file in classic mode "Attachment" trigger gets executed and in Lightning Mode "ContentDocumentLink" Trigger gets executed.
Try this for Lightning Mode:

I think the issue is that you are using the Create operation for the action. Instead of creating a new case, you should use an Update operation. As the form says:
If the CRUD Operation is Update an ID field mapping is required.
I can't see that you have included an ID field in your field mappings there either. You will need that ID, so that you can update your customer.
In system I am developing, user can access particular page which is used to make DML operation on custom object. I want to fulfill following business case :
User A opens a Page made changes on page but did not click Save.
At the same time User B opens a page made changes and click Save. User B data got saved in record.
Now user A came and click Save. But as data is outdated I want to prompt user that "you are over righting the latest update. Do you want to continue."
Is there any faster out-of-box way available to achieve it ?
Assuming this is in a VisualForce page, you could check if the value has changed inside your Save method.
Query for the LastModifiedDate of all records you may change, and save that in your controller. This should be done in your controller's constructor and/or anywhere you re-load the record(s).
In the save method, first query the record(s) again to check if the LastModifiedDate has been changed. If it has been changed, prompt the user.
Assuming you are trying to insert a record into an object.
You can write a validation rule on that object. Check if that record is already inserted or not and show an error message.

i'm developing an ADF application. I have a problem. My Jdev version 11.1.2.3.0.
So, For example i have an URL like this "http://www.hohoho.com/view/index.xhtml?_adf.ctrl-state=ju9lnu5ld_3"
In this page i have a table gets value from DB. For example again, i changed some rows in DB browser, and clicked the web browser's refresh button. But new results doesnt get!. For example i remove the "?_adf.ctrl-state=ju9lnu5ld_3" and enter url, this gets new results. How can handle this situation. I need that when an user clicked the refresh button, last result must be fetched. I think it is based on ADF state. How can handle this situation.

Thanks Andread, mysql's autocommit property default value is true, not false :) but i was using it false. I've solved the problem like yours, but my own solution so cool :) I've only overried the clearCache() method. And it solved the problem.

The results from the query are cached in the middle tier. This is the reason why updates to the database which are applied through a different channel than through your application are not reflected when the page reloads.
I am using JDeveloper 11.1.1.7. Setting the "CacheResults" property on the Iterator to false solved this issue (go to the "Bindings" tab of your page or page fragment which contains the table, select the Iterator Executable for your table data, and in the Property Inspector at the "Advanced" section, set "CacheResults" to "false").
In terms of XML, the iterator definition in the PageDef.xml file should look like

DataControl="AppModuleDataControl" RangeSize="25"
CacheResults="false"/>

OP uses MySQL. With MySQL, setting the CacheResults property is required, but not sufficient. By default, MySQL runs with autocommit=false which has the side effect of using the isolation level REPEATABLE READ. A SELECT implicitly opens a transaction, and subsequent SELECTs return the same result. The Oracle RDBMS uses READ COMMITTED by default, so that data inserted and committed in one session is returned by SELECT in a different session.
One solution to get around this in ADF is to create an implementation class for the View Object, override executeQueryForCollection() and commit the transaction before executing the query:

Please use this carefully and review your actual isolation level requirements to make sure that you do not unintentionally commit data by a browser refresh. Another drawback of this solution is that it is not portable between Oracle RDBMS and MySQL.

I cannot find where in the salesforce.com UI I can add a trigger on a file attachment. I can find triggers on almost everything else, but attachment seems to be missing from the list (even when I view source on the page and search it. Does anyone know WHERE I can put this trigger in?
There is no way to do it directly as Attachment is one of those "lesser" objects that salesforce really gets "protective" about in a random and biased way. The only "legit" way to do it is to use some external build&deploy tools such as Force.com IDE.
If however you are not a stranger to undocumented 'hacks' do the following. Go to any object's trigger list and click create new. In the URL locate entity query string parameter (e.g. entity=Case) and change it to Attachment (entity=Attachment) and press Enter. Newl loaded screen will accept Attachment trigger.
Using Eclipse (Force.com IDE plug-in), right-click on your project & select "New" > "Apex Trigger". In the dialogue window that pops up, there is an "Object" dropdown / picklist, choose the object you want from this list - "Note" or "Attachment" etc. - then choose the "events" you want to trigger to execute on.
NOTE: a best practice is only one (1) trigger per object since you cannot guarantee the order in which multiple triggers on the same object will execute.

In my opinion it should be possible from there.
To write a trigger for attachment, there is no straight way to do it but you can do it.
By creating a trigger on other object for example create a trigger in contact object then the url of the current page shows
"https://ap1.salesforce.com/........./&entity=Contact" you have to change "entity=Attachment"
then the trigger will be created for attachment object.
Here is the scenario:
I have a winforms application using NHibernate. When launched, I populate a DataGridView with the results of a NHibernate query. This part works fine. If I update a record in that list and flush the session, the update takes in the database. Upon closing the form after the update, I call a method to retrieve a list of objects to populate the DataGridView again to pick up the change and also get any other changes that may have occurred by somebody else. The problem is that the record that got updated, NHibernate doesn't reflect the change in the list it gives me. When I insert or delete a record, everything works fine. It is just when I update, that I get this behavior. I narrowed it down to NHibernate with their caching mechanism. I cannot figure out a way to make NHibernate retrieve from the database instead of using the cache after an update occurs. I posted on the NHibernate forums, but the suggestions they gave me didn't work. I stated this and nobody replied back. I am not going to state what I have tried in case I didn't do it right. If you answer with something that I tried exactly, I will state it in the comments of your answer.
This is the code that I use to retrieve the list:

The session is passed to the class when it is constructed. I can post my mapping file also, but I am not sure if there is anything wrong with it, since everything else works. Anybody seen this before? This is the first project that I have used NHibernate, thanks for the help.
After your update, Evict the object from the first level cache.

You may want to commit and/or flush first.
